We help clients create a care plan after the diagnosis of a terminal illness. Each plan is customized to fit the client’s needs and desires. We make sure to include safety, hygiene, activities of daily living. Our goal is make sure your loved one feels more supported and empowered to live life fully. 

In-home evaluations addressing quality of life, daily activities and personal care

Create a plan of action to ensure clients treated with dignity and respect

Facilitate discussions between family members allowing them to share concerns and fully understand the situation

Act as a resource guide for families as they build their care team 

Care Partner Education & Training

Being a care partner for a loved one is often challenging, confusing, and exhausting, but it doesn’t have to be. I can equip you with tools and techniques for care-giving and be an encouraging advocate alongside you and your loved one every step of the way. To help make both yours and your loved one’s journey easier, I will:

Educate family and friends to help them understand the disease, what it involves, and how they will be impacted

Teach Positive Approach to Care ™ techniques to family members and care partners, professional staff, and first responders

Act as an encouraging advocate alongside you and your loved one every step of the way. To help make both yours and your loved one’s journey easier, I will:

Engage with clients on doctor visits, during social activities, and using intellectual stimulation

End-of-Life Coaching

Let’s talk openly about end-of-life issues. I am a certified End-of-Life Coach: someone who is trained and experienced in noticing end-of-life transitions, non-verbal cues, and creating a safe and meaningful experience. Together, let’s create a pathway that ensures all legal, care, and memorial planning logistics are covered so that you and your loved one can focus on what truly matters. I will help you navigate this path by providing:

• Education on the different levels of palliative and hospice care

• Decision-making tips about maintaining your loved one’s quality of life

• Assistance in making funeral arrangements, memorial services, or celebrations of life

• Help for the individual dying (i.e. providing their “voice”) and offering compassionate care and support to family/friends
